I read the interesting study on correlation of thyroid antibodies and neurologic manifestations specially the encephalopathy, psychosis, refractory seizures, etc.1 The simultaneous editorial on the same issue is also enlightening.2 The authors concluded that Hashimoto encephalopathy requires redefinition. Here, we would like to share our small but important experience. We found in a cohort of We thank Dr. Shubhakaran for the comments on our article1 that emphasize the risk of giving relevance to thyroid peroxidase (TPO) antibodies when they are probably irrelevant. Shubhakaran et al.2 found a TPO antibody frequency of ∼2.8% in their series of ∼70 patients with myelopathies and neuromyelitis optica spectrum disorders (NMOSD).

Why write about hepatic encephalopathy (HE) in the twenty-first century? It is in front of this question that we found ourselves, once invited to serve as guest editors for the Special Issue of the Journal of Clinical Medicine entitled “Hepatic Encephalopathy: Clinical Challenges and Opportunities”. OBJECTIVE: To report the presenting syndromes and to determine whether pretreatment criteria of Hashimoto encephalopathy (HE) predict response to steroids. METHODS: We assessed symptoms and steroid responsiveness in 24 patients with pretreatment criteria of HE, including (1) subacute onset of cognitive impairment, psychiatric symptoms, or seizures; (2) euthyroid status or mild hypothyroidism
Hashimoto encephalopathy has also been termed steroid responsive encephalopathy associated with thyroid autoantibodies (SREAT), and many investigators prefer this descriptive term (Castillo et al., 2006).
